Your First Visit

We believe quality dental care depends on a strong doctor-patient relationship founded upon mutual respect and open communication. Your first visit is a great time for us to sit down and get to know each other. We want to know what you do for a living, what your hobbies are, and how your daughter did in her last swim meet.

We also want to know about your diet and lifestyle, and your medical and dental history including any medications you are taking. We want to know what your oral and overall health goals are so we can, together, come up with a plan to get you there.

Your first visit will include a comprehensive exam, necessary x-rays, and a review of your treatment options. In many cases, we can begin the treatment process the same day as your initial visit. For more complex medical histories and treatment plans, you may need to schedule a second appointment to begin treatment.

To make your first visit as smooth as possible, please bring all of the following that apply:

  • X-rays: X-rays taken by your previous dentist. You can ask them to forward your x-rays to our office, or you can pick them up and bring them with you. We will take additional x-rays as needed.
  • List of Medications: A list of medications you are currently taking.
  • Dental Insurance: If you have dental benefits, please bring your insurance card with you and any necessary forms.

All new patient, medical and dental history forms are completed electronically in-office. Please arrive at least 10 minutes early to complete your forms.

Office Amenities

You will always be greeted with a smile and offered one of our assorted complimentary refreshments. We know you are busy, so we offer family appointment blocks for your convenience, and always strive to keep appointments running on time. Our treatment rooms are comfortable and relaxing. You can listen to music or watch TV with our noise-cancelling headphones to make your treatment a breeze.

Treatment Room

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian. Please tell us if you have a health problem that may adversely affect your treatment, like high blood pressure, rheumatic fever, diabetes, or artificial heart valves and joints. Also, let us know if you are currently taking any medication, including aspirin, heart medications, anticoagulant therapy, etc.

Infection Control

At Modern Smile Center, we take infection control and sterilization techniques seriously to protect you and our team. We follow procedures recommended by the Occupational Safety & Health Administration (OSHA), the Environmental Protection Agency (EPA), and the Centers for Disease Control (CDC). These measures include:

  • Using disposable materials
  • Sterilizing all equipment before each use
  • Chemically disinfecting countertops and surfaces
  • Wearing gloves and face masks
  • Washing with disinfectant hand soap

We sterilize all reusable office equipment using an autoclave that kills bacteria and viruses with steam, heat, and pressure.

Financial Policy

Payment is due when services are scheduled unless other arrangements have been made in advance. If you are unable to pay at the time of scheduling, we will collect a down payment for larger appointments. For smaller procedures, we will collect payment at the time services are rendered. You are responsible for all fees.

Each month, we will send you a financial statement for accounts overdue. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time period. Your remaining balance after your insurance payment is your responsibility. If necessary, we can make arrangements for a monthly payment plan, but we must determine your plan before beginning treatment.
